
WHAT IS A MEDICAL RECORD? A patient’s medical record is a summary of his/her medical history and the treatment they received. For the patient’s protection, these guidelines include specific requirements to obtain medical records.
For a patient to obtain his/her records, GCH requires that the patient, their guardian or legal representative submit a request for the record in person, in writing and complete an authorization form. This form allows Medical Records to obtain the patient’s information.
Patients must supply their name, Social Security Number and date of birth. The written request must also indicate where to send the records and be signed and dated by the authorized requestor. If available, the date of service should also be included. Please bring a valid photo identification with you.
WHAT IF SOMEONE OTHER THAN THE PATIENT NEEDS A COPY OF THE RECORDS? Medical information is private and confidential. In compliance with federal government Health Insurance Portability and Accountability Act, GCH will not release records without proper authorization from the patient, their legal guardian or legal representative.
WHAT IF THE PATIENT CHANGES PHYSICIANS? Patients can get copies of their medical records sent to a different health care provider. There is no charge for this service. The patient, their guardian or legal representative will need to mail or fax a written request or complete an Authorization to Release form and submit to Medical Records to be processed. There is no charge to have medical record copies sent directly to another health care provider for patient’s continued health care.
